Health Coaching

The Health Coaching system supports call centre activity and proactive health coaching of chronically ill patients towards healthier living habits, better medication compliance and better self-management of their condition at home. Patients go trough a tailored and structured training program for 6-12 months, and after having completed the programme are able to self-manage their condition better and are less likely to develop more severe complications.

Individually allocated health coaches (by health service providers) offer support for patient self-management while information sharing and the reduced need for clinical visits help doctors and nurses provide better care. The objective is to prevent patients from developing compilications and to decrease morbidity and hospitalization.

The Health Coaching system is a separate module in ProWellness Chronic Diseases Management System (CDMS) with additional functionality to support health coaches' work. The module enables information sharing between health coaching units and other hospital or community clinics using ProWellness CDMS. The training programmes can be trailored as per needs by the health services providers.

Health Coaching was originally launched in the city of Lahti as a joint project by ProWellness, Pfizer Health Solutions, Sitra, Tekes and the Päijät-Häme Federation of Social Services; in a two year study health coaching was offerered to patients who suffered from type II diabetes, cardiac insufficiency or arteriosclerosis.
