ProWellness Cardiac system is part of our Chronic Diseases Management System (CDMS) platform and has been designed in co-operation with clinicians for managing patients with cardiac conditions, such as e.g. heart failure, throughout the Care Pathway. The software uses individually designed pages to meet the needs of healthcare providers, covering all stages of cardiac diseases whether in hospital or community environment, including rehabilitation and palliative care.
The system allows clinical end-users to easily collect, create, and submit data to monitor clinical outcomes and activity at the care unit and/or organisational level.
The ProWellness Cardiac system can also generate audits and reports that adhere to national audit guidelines such as the British National Heart Failure Audit as specified by NICOR (National Institute for Cardiovascular Outcomes Research, University College London).
