The ProWellness Diabetes Management System is a regional web-based information system for the care and management of diabetes. The system is used by doctors, nurses, dietitians, podiatrists and other healthcare professionals involved in the diabetes care process. The solution can be used in one or more care units and can be rolled out step by step to cover the entire health district. The system supports information sharing and cooperation between primary and secondary care and patients at home.
The system enables the care team to easily obtain a comprehensive view of the patient’s current condition, development and care history. Reports and summaries are displayed according to defined care objectives and the development of a patient’s condition over a period of time is highlighted using colours and graphs. Active use of the system enables more focused care and resource allocation on patients with an increased risk of developing complications and related illnesses.
Information stored in the system can be utilised in care planning and monitoring of care quality. The reporting tool produces instant feedback on health care professionals’ own work. Reports can be generated to measure and compare care quality at care unit, district and national level.
