Chronic Disease Management System

 

ProWellness develops regional web-based information systems for the prevention, care and management of long-term conditions such as diabetes, cardiovascular diseases, COPD and asthma. The ProWellness Chronic Disease Management System (CDMS) is a comprehensive solution for healthcare professionals within a healthcare region for daily care of patients with chronic conditions. The web-based system is simple, intuitive and user-friendly and it offers excellent tools for tracking patient status and care outcomes. All data in the system can be used for quality assurance and audit purposes.


The CDMS supports multiple chronic diseases and can be used in private, primary and secondary care environments. Currently available modules are Diabetes Management, Retinal Screening, patient SelfCare and Diabetes Prevention. Modules for cardiovascular diseases and COPD are under development. In addition to disease management, the system supports patient empowerment and preventive care as well as promotes patient’s wellbeing leading to cost savings in healthcare organisations.

 

Information content in the system is based on evidence based medicine (EBM); evidence based nursing (EBN) and the latest care guidelines and standards. In order to ensure the system’s compatibility with prevailing work patterns, ProWellness works closely with care professionals and patients.


The CDMS integrates with other clinical and management systems, such as PAS, pathology, and primary care systems.  The ProWellness Platform is EHR and EPR compliant and it is built upon proven technological solutions. It is compatible with major healthcare industry standards: HL7, CDA, XML, ICD10, SNOMED, READ and ASTM. The ProWellness platform uses standard relational ORACLE™ database solutions and data management tools.


The CDMS is an EHR system for management of long-term conditions. The system complements traditional EPR systems by providing disease specific guidance and support in daily care of patients with chronic diseases. Data accumulated in the system can be used for quality and performance monitoring and operations planning purposes.
